Pursuant to O.R.C. §1701.87, Notice is hereby given that PLASTIC SURGERY CONSULTANTS, INC., has elected to dissolve. A Certificate of Dissolution will be filed with the Ohio Secretary of State with an effective date of dissolution of May 31, 2023. All creditors of and claimants against Plastic Surgery Consultants, Inc. must present detailed claims and demands within sixty (60) days of the effective date of this notice, in writing to the corporation, 3645 Corey Rd., Toledo, OH 43615. Claims not received by the deadline will be barred.
